TRxANSITION Index™: A Provider-Administered Readiness Assessment
The TRxANSITION Index™ is a comprehensive 32-item questionnaire spanning 10 domains of transition readiness. Administered by trained professionals in approximately 7-8 minutes, either in person or remotely, this tool offers a structured approach to evaluating patient needs. To further enhance patient understanding and engagement, TRxANSITION Index™ Educational Handouts are available, aligning with each subdomain to provide targeted education. This ensures that patients receive tailored information to address their specific transition challenges.
STARx Questionnaire: Self-Assessment of Transition Preparedness
For patient self-assessment, the STARx Questionnaire provides an efficient 18-item tool across 3 key domains. Requiring only about 3 minutes to complete on paper or computer, this self-report questionnaire offers a quick snapshot of an individual’s perceived transition readiness. It serves as a valuable starting point for discussions between patients and providers, highlighting areas where additional support may be beneficial.
Transition Passport™: A Portable Health Summary for Seamless Communication
The Transition Passport™ is a practical, wallet-sized health summary designed to facilitate communication and patient education. This portable tool empowers patients to carry essential medical information with them, ensuring easy access for both themselves and their healthcare team.
